Important: Verify your membership to access this form
Step 1: Enter the email address you signed up with and your PRDJ / RRDJ number.
A security code will be sent to your registered email address.
Step 2: Copy the security code from your email and enter it into the
Verification Code field, then click Verify Code.
Step 3: Once your membership has been successfully verified, the form will appear
underneath this box with your membership details entered.
If you cannot see the email or no longer have access to your registered email address,
please contact your Steward for help. You can also
